Insurance Information
Our office is in-network with Delta Dental Premier. We gladly submit to all PPO insurance companies as an out-of-network provider.
Our out-of-network patients tend to be happy with their coverage for routine services such as cleaning, exams and x-rays. Our staff is more than happy to answer any questions you may have regarding out of network insurance companies.

Our office is located at 500 Primrose Road, Suite 2 Burlingame, CA 94010. Click here to get directions.
We have a small parking lot on the Primrose Road side of the building, as well as plenty of street parking around the office.
We are located in downtown Burlingame, and are accessible by Caltrain.
